Once you have scheduled your initial visit at our Denver chiropractic office you will receive an email link that will allow you to log in and fill out some introductory paperwork. Once you complete the online patient information, the office will be notified and you will be contacted with any additional questions. 

If you are not able to complete the paperwork ahead of time, please plan to arrive 10-minutes early to your first visit as we will not be able to start your appointment until it is completed.
